SQL Yearbook by Jen McCown of MinionWare!

We're finally moving forward on the SQL yearbook idea. This is for all community members who want to participate...but you have to opt in (here)! 
 
  • Deadline: Tentative deadline to be a part of this is October 1. Sign up before then!
     
  • How your data is used: The information you provide in the form below (and the linked form) will not be sold or provided to anyone else (except of course for the completely public yearbook that anyone will be able to download, but THAT won't contain your email address). We won't add you to any newsletters, or provide sponsors with a list of contacts or anything like that. All information is for the SQL Yearbook, and to allow us to contact you about the SQL Yearbook. 
     
  • Permission: By completing this form, you agree that this information (except your email address) may be published in a book that could be sold as a hard copy. For example, on Amazon.com. The proceeds from the sale of the book WILL go to a non-political charity (e.g., something like Doctors Without Borders or a children's hospital).
